Getting Started
Animal Hospital is a multiplayer horror game where you run a hospital for animals — but some patients are dangerous anomalies in disguise. Up to 4 players work together to treat real patients while identifying and rejecting anomalies.
First Steps
- Choose the Intern class — it's free and gives +10 bonus sanity
- Grab Coffee immediately at the start of every shift (5 Animal Coins, restores 20 sanity)
- Check in patients at the reception desk
- Check for anomalies before admitting anyone — this is your most important job
How to Check for Anomalies
Always follow this 3-step check for every patient:
Step 1: In-Person Visual Check
Look directly at the patient in the waiting room. Reject immediately if you see:
- Three eyes
- Hollow / black eyes
- Sharp or realistic teeth
- Wide creepy grin
- Twitching limbs or neck
- Mismatched eyes
Step 2: CCTV Camera Check
Switch to the CCTV monitor. Reject if you see:
- Black censor bars over eyes
- Distorted / stretched body
- Void / completely black body
- Patient staring directly at camera
- Camera zooming toward a dark figure (look away fast — drains sanity!)
Step 3: Photo Check
Take a photo of the patient and check for:
- Photo shows different animal or features than the patient in person
- Static / grainy photo
- Cursed photo (bloodshot eyes + grotesque grin) — do NOT pick up (drains 10 sanity)
How to Reject
Press the Shutters button on the left side of your reception desk once you're sure it's an anomaly.
Treating Patients

Tip: Check in ALL patients first, then treat them. Recovery happens automatically.
Sanity Management
Your Sanity is your most important resource. If it hits zero, your run ends.

Sanity Recovery by Class
- Secretary: Recovers sanity when checking in patients
- Doctor: Recovers sanity when healing patients
- Surgeon: Recovers sanity + speed boost when healing
- Psychologist: Double sanity effects (good AND bad) — high risk!
Playing with a Team
Animal Hospital supports up to 4 players. Divide roles:
- Receptionist: Checks in patients and screens for anomalies at the front desk
- Doctor: Treats patients in medical rooms
- Camera Operator: Monitors CCTV for anomalies and threats
- Runner: Handles emergencies and delivers items between rooms

- Always buy Coffee at the start of every shift
- Play with sound on — Skinwalkers roar when they emerge, Hiders breathe loudly
- Check CCTV frequently — some anomalies are camera-only
- Always carry Maple Syrup from Shift 3 onward (for Bed Monsters)
